Mutual Fund Forms: What Each Form Is Used For

Every mutual fund form is either Financial (it moves money or changes an investment) or Non-Financial (it only changes or records account details). Fund houses (AMCs) name the same form differently, so this article lists the common aliases for each form.

How to read this article

  • Financial forms: used to invest, start or stop a SIP/STP/SWP, switch, redeem or register a payment mandate.

  • Non-financial forms: used to update nominee, bank, contact, name, signature, tax status or KYC details, or to claim units after a holder's death.

  • A few forms (for example the Common Transaction Form and SIP Cancellation Form) are filed as Financial by some AMCs and Non-Financial by others.

  • Always confirm the exact required documents on the AMC's own form. This article explains the purpose of each form, not the AMC-specific checklist.

Financial forms

Financial forms are used when money moves or an investment instruction changes.

Which forms an NFO investor needs

Investor situation

Forms required

New investor, no folio, lump sum

The scheme's application form (or the Common Application Form, CAF), plus KYC and bank proof.

The scheme's application form or CAF with the SIP section, a SIP Form, and the NACH/OTM bank mandate.

Existing investor, SIP in the NFO

The SIP Form plus a NACH/OTM mandate.

Investing through a platform (MFU, BSE StarMF, NSE)

The Switch section of the Transaction Slip.

Payment made from someone else's bank account

Add the Third-Party Payment Declaration Form.

Minor investor

New Karta appointed after the Karta's death (T4)

Annexure 6

Bond of indemnity by the claimant

HUF dissolved or no surviving members after the Karta's death (T5)

AMC-specific numbering and the scenarios where each annexure is required vary. Some AMCs number the annexures differently, and the fund house's transmission matrix (documents ready reckoner) is the final reference.

Points that commonly come up

  • A registered nominee usually does not need the indemnity bond, affidavits or NOCs. The claim then needs only the nominee's own documents.

  • Common supporting documents: certified death certificate, claimant's PAN and KYC, a cancelled cheque or bank statement in the claimant's name, and a nomination form signed by the claimant.

  • Larger claims need additional formalities, such as an attestation before a notary or Judicial Magistrate. AMCs apply value thresholds; the commonly cited one is ₹5 lakh.

  • A minor claimant files through a guardian and also needs a birth certificate.

  • An Intimation of Demise letter can be used first to report the death to the AMC.

Quick answers

Q: What forms are required for transmission? Transmission needs one main form depending on the case, plus annexures. T1 removes a deceased joint holder's name. T2 is for surviving joint holders after the first holder dies. T3 is for a nominee or legal heir when the sole or all holders have died. T4 is for a change of Karta in an HUF. T5 is for HUF dissolution or settlement. Annexures 1A to 6 are the bank attestations, indemnity bonds, affidavits and NOCs that go with them.

Q: What is Form T3 used for? A nominee, legal heir, successor or administrator uses T3 to claim units when the sole holder or all joint holders have died.

Q: What documents go with a nominee's transmission claim? A certified death certificate, the claimant's PAN and KYC, a cancelled cheque or bank statement in the claimant's name, and a signed nomination form. Indemnity bonds and affidavits are usually not needed when a nominee is already registered.

Q: What forms are required to add or change a nominee? The Nomination Form. To say you do not want a nominee, use the Opting Out of Nomination Form.

Q: What forms are required to change my bank account? The Change of Bank Form, with bank proof and, where required, a bank attestation of the signature. To add another account without removing the old one, use the Multiple Bank Accounts Registration Form.

Q: How do I update my email, mobile number or address? Use the Contact Updation Form (also called Email Id / Mobile Updation Form or Change in Contact and Address Form).

Q: Which form changes my name or signature? Use the Name Change Form for a name change (including after marriage) and the Signature Updation Form for a signature change. Both are non-financial.

Q: Which form do I use when a minor turns 18? The Change in Status from Minor to Major Form.

Q: Which form starts a SIP? The SIP Form, together with a SIP NACH / OTM form for the bank auto-debit. A new investor uses the Common Application Form.

Q: Which form stops or pauses a SIP? Use the SIP Pause Form to pause and the SIP Cancellation Form (or SIP/STP/SWP Cancellation Form) to stop it permanently.

Q: Which form do I use for STP or SWP? The STP Form moves money between schemes on a schedule. The SWP Form pays out a fixed amount regularly.

Q: Which form do I use to invest more in an existing folio? The Transaction Slip.

Q: Which forms are needed to switch distributor or move from regular to direct? The Change of Broker Form. Use the EUIN Remediation Form if the EUIN is missing or wrong.

Q: What are FATCA, CRS and UBO forms? Tax-residency and ownership declarations required by regulators. FATCA/CRS apply to individuals and entities. UBO applies to entities.

Q: Which form combines two or more of my folios? The Folio Consolidation Form.

Q: Which form is used when someone else pays for my investment? The Third-Party Payment Declaration Form.

Q: What is the difference between a financial and a non-financial form? A financial form moves money or changes an investment (purchase, SIP, STP, SWP, redemption). A non-financial form changes account details (nominee, bank, contact, name, KYC).

Q: What forms are required for an NFO? A new investor fills the scheme's NFO application form (or the Common Application Form) with KYC and bank proof. An existing investor can use the same form or a Transaction Slip with the folio number. A SIP in the NFO adds the SIP Form and a NACH/OTM mandate. Some AMCs use one form for all schemes and others use one per NFO, so confirm on the AMC's offer document.

Q: Is there a separate form for each scheme? Many AMCs publish a scheme-specific form (for example Small Cap Fund Form, Flexi Cap Fund Form, Multi Asset Allocation Fund Form). Others use the Common Application Form for every scheme.

Q: Which form do I use to invest in an index fund or ETF FoF NFO? The index fund or ETF FoF form published by that AMC (for example the Nifty India Defence Index Fund Form). Existing investors can use the Transaction Slip.

Q: Which forms are needed for a children's or retirement fund? The scheme's own form (for example Children's Fund Form or Retirement Savings Fund Form). Children's schemes also need the minor's and guardian's details and KYC.

Q: What are SIF forms? Forms for Specialised Investment Funds, such as Equity Long Short or Hybrid Long-Short funds. They are scheme-specific and usually have a higher minimum investment than regular funds.

Q: Which form do I use to start a SIP in a new fund? The scheme's application form or CAF for a new investor, or the SIP Form for an existing folio. Add a NACH/OTM mandate for the bank auto-debit.

Q: Which form switches money from one scheme to another? The Switch section of the Transaction Slip. Systematic switching uses the STP Form.

Q: Which form redeems units? The Transaction Slip (redemption section). A regular payout from an investment uses the SWP Form.

Q: How do I register more than one SIP at once? Use the Multi SIP Form (also called Multiple SIP, Multi Goal SIP or Combo SIP Form).

Q: Which form do I use for a fixed deposit? The FD form of the issuing company or bank, in the version for individual, NRI, corporate or trust applicants. Renewals and KYC have their own forms.

Q: Which forms does an NRI need? The NRI Form or Tax Status Change Form to record NRI status, plus FATCA/CRS and KYC forms.

Q: Which forms are needed for a company, trust or HUF investor? The non-individual KYC form, FATCA/CRS Non-Individual form and UBO form, plus the scheme or Common Application Form.

Q: What if my payment comes from a different bank account than the one on record? Submit the Third-Party Payment Declaration Form, or the Multiple Bank Accounts Registration Form to register the new account first.

Q: What is the difference between the Common Application Form and the Transaction Slip? The Common Application Form opens a new folio and records full investor details. The Transaction Slip is only for a transaction in an existing folio.

Q: Which form updates PAN or KYC details? The PAN KYC Updation Request Form, KYC Modification Form or ISR-1, depending on the AMC and registrar.

Q: Which form do I use if a payout cheque was lost or expired? The Cheque Reissuance Form.
